The Centre for Self Management Support is located within Cambridge University Hospitals NHS Trust.
The Centre supports patients with multiple long term conditions (including lung disease, diabetes, stroke rehabilitation, oncology and palliative care) to successfully manage their own health.
The team combines the skills of clinicians, managers and researchers and continues to involve patients at every stage of its development. They rigorously evaluate their work and are busy publishing results of their trials to share their learning with others.
The team takes an integrated, holistic approach, recognising that patients develop expertise in managing their condition in a manner that meets their needs, and are not merely passive recipients of care.
